If you take medications for a variety of different medical conditions, you may be eligible to get services through MedicoRx® Specialty Pharmacy Medication Therapy Management (MTM) Program. This program helps you and your healthcare provider make sure that your medications are working to improve your health.
If you qualify for our service, we will mail you an introductory letter along with your welcome packet that explains our MTM Program in detail along with a form that you can use to sign up for the program.

Shortly thereafter, you will be contacted by our MTM Pharmacist who will discuss the program further and will conduct an initial evaluation.

Comprehensive Medication Review:
A licensed and experienced pharmacist will give a comprehensive review of all of your medications and discuss with you:
You will get a written summary of our discussion. This summary will include a Medication Related Action Plan and a pre-filled Medication Record Form to help you keep track of the medications that you are taking and why you are taking them. We will share the Medication Related Action Plan with your healthcare provider to assure that all necessary issues are addressed.
Quarterly Medication Review:
Our pharmacist will then follow with a Targeted Medication Review to address your Medication Related Action Plan and assure that:

Currently our program is open to all patients with chronic disease who are taking two or more medications. There may be additional criteria that can qualify you for our MTM Program. Shortly thereafter, you will be contacted by our MTM Pharmacist who will discuss the program further and will conduct an initial evaluation.
A Comprehensive Medication Review is a one-on-one consultation with our knowledgeable and experienced pharmacists. During the process, the pharmacist will review all the medications that you use, including prescriptions, over-the-counter medications (OTC), and herbal supplements. The pharmacist will also review and document the purpose and directions for each medication on the Medication Record Form. You may also take this time to discuss disease-specific goals of therapy, any medication-related or program specific questions you may have. Please allow at least 30 minutes for your Comprehensive Medication Review. The review may take longer depending on the circumstances surrounding your case. After the Comprehensive Medication Review, you will receive a follow-up letter that includes a Medication Related Action Plan along with Medication Record Form. You will have an initial Comprehensive Medication Review and an annual one, if applicable.
Quarterly Targeted Medication Reviews are conducted systematically to identify any drug-drug interactions or other medication concerns. Those results may be mailed to your doctor for review. Patients who complete a Comprehensive Medication Review are eligible to receive a 15-minutes interactive Quarterly Medication Reviews in addition to the initial review. This time may be used to discuss any concerns you have with the pharmacist.
