MedicoRx® Specialty Pharmacy has created a Patient Management Program, which utilizes the application of evidence-based medicine as appropriate. The program is headed by an MTM-certified pharmacist and supported by appropriately trained pharmacy, billing, and IT staff. Our prime goal with this program is to optimize therapeutic outcomes through improved medication use and reduce the risk of adverse events. We strive to assure that medication prescribed to you is readily accessible and that your needs are monitored to achieve the best possible result thereby minimizing or avoiding adverse results. Our goal is achieved through education and counseling of patients and caregivers where applicable, using the following service model:

MTM Service Model

All Requests Addressed Within 24 Hours
In order to best serve your immediate needs, ALL requests are addressed within 24 hours. When a request for clinical information is received, our trained customer service staff refers the patient/customer to the appropriate clinician who will then attempt to address the customer’s clinical concerns. If the clinician is unable to address the customer’s concern, it will be forwarded to the Clinical Senior Leader who will address the issue in a timely fashion.

  • Answer concerns about medication costs and drug reactions;
  • Find the way to best take your medications;
  • Address any questions or concerns you have about your prescription and over the counter medication.

Our MTM Program will help PROVIDERS

As health care providers, pharmacist share your goal of helping patients to achieve the best possible outcome from their prescribed therapy.  Pharmacists are ideal partners to work with prescribing therapy.  Pharmacists are ideal partners to work with prescribing practitioners to meet this objective by working with patients to increase their understanding of the drugs that were prescribed, and their role in getting the best possible results.

Core Elements of Patient Management MTM at MedicoRx® Specialty Pharmacy:

The MTM certified pharmacist and staff will engage in a systematic process of collecting patient-specific information, assessing medication therapies to identify medication-related problems, prioritizing the problems, and creating solutions.

The information collected may include the following:

  • Interviewing the patient to gather data including demographic information, general health and activity status, medical history, medication history, immunization history, as well as the patient’s own comments about his/her condition and medication use;
  • Assessing, on the basis of relevant clinical information available to the pharmacist, the patient’s physical and overall health status, including current and previous diseases and conditions;
  • Assessing the patient’s values, preferences, quality of life, and goals of therapy;
  • Assessing cultural issues, education level, language barriers, literacy level, and other characteristics of the patient’s communication abilities that could affect outcomes;
  • Evaluating the patient to detect symptoms that could be attributed to adverse events caused by any his or her current medications;
  • Interpreting, monitoring, and assessing patient’s laboratory results;
  • Assessing, identifying, and prioritizing medication-related problems related to:
    • The clinical appropriateness of each medication being taken by the patient, including benefit versus risk;
    • The appropriateness of the dose and dosing regimen of each medication, including consideration of indications, contraindications, potential adverse effects, and potential problems with concomitant medications;
    • Therapeutic duplication or other unnecessary medications;
    • Adherence to therapy;
    • Untreated diseases and conditions;
    • Medication cost considerations;
    • Healthcare/medication access considerations
  • Developing a plan for resolving each medication related problem identified;
  • Providing education and training on the appropriate use of medications and monitoring devices and the importance of medication adherence and understanding treatment goals;
  • Coaching patients to be empowered to manage their medications;
  • Monitoring and evaluating the patient’s response to therapy, including safety and effectiveness;
  • Communicating appropriate information to the physician or other healthcare professional on issues such as selection of medications, suggestions to address identified medication problems, updates on the patient progress, and recommended follow-up care;

MedicoRx® Specialty Pharmacy strives to promote medication self-management among its patients. The following information may be collected to assist the patient with medication self-management through the MTM program:

  • Patient name;
  • Patient date of birth;
  • Patient phone number;
  • Emergency contact information;
  • Primary care physician information;
  • Pharmacy/pharmacist information;
  • Allergies information;
  • Other medication-related problems;
  • Potential questions for patients to ask about their medications;
  • Date last updated;
  • Date last reviewed by the pharmacist, physician, or other healthcare professional
  • Patient’s signature;
  • Healthcare provider’s signature;
  • For each medication include the following:
    • Medication (e.g. drug name and dose)
    • Indication (e.g., Take for…)
    • Instructions for use;
    • Start date;
    • Stop date;
    • Ordering prescriber/contact information;
    • Special instructions;

The use of this information in the appropriate format will promote adherence as well as communication and collaboration with physicians and other healthcare professionals to achieve optimal patient outcomes.

Additionally, MedicoRx® Specialty Pharmacy collects data to create a Medication Related Care Plan that promotes self-management and further promotes the goal of a patient-centric Patient Management Program. Mediation Related Care Plan, coupled with education, is an important element in promoting a sense of patient empowerment and encourages patient participation in his/her medication-adherence behavior and overall MTM. The Medication Related Care Plan may include the following information:

  • Patient name;
  • Primary care physician;
  • Pharmacy / pharmacist;
  • Date of plan creation;
  • Actions steps for the patient (problems identified);
  • Notes for the patient (including goals and interventions);
  • Appointment information for follow-ups.

At this time, MedicoRx® Specialty Pharmacy does not provide referral services; however, an MTM-certified pharmacist provides consultative services and intervenes to address medication related problems. Suggestions may be made by the pharmacist for follow-up care. The intervention promotes collaboration between the patient, the patient’s physician, and other healthcare providers. Examples of circumstances that may require referral may include the following:

  • A patient may exhibit potential problems discovered during the MTM that may necessitate referral for evaluation and diagnosis;
  • A patient may require disease management education to help him or her manage chronic diseases;
  • A patient may require monitoring for high-risk medications;

The intent of the intervention, if such becomes feasible, is to optimize medication use and adherence, enhance continuity of care, and encourage patients to avail themselves of healthcare services to prevent future adverse outcomes;

Documentation is an essential element of MTM services. MedicoRx® Specialty Pharmacy assures that MTM services are properly documented to assure that our patient-related needs are being met. MTM documentation includes creating and maintaining an ongoing patient-specific record that includes a record of all care provided to the patient and may serve several purposes including but not limited to:

  • Facilitating communication between the pharmacist and other healthcare providers;
  • Improving patient care and outcomes;
  • Enhancing the continuity of patient care among providers and care settings;
  • Ensuring compliance with laws and regulations for the maintenance of patient records;
  • Protecting against professional liability;
  • Capturing services provided for justification of billing and reimbursement;
  • Demonstrating the value of pharmacist – provided MTM services;
  • Demonstrating clinical, economic, and humanistic outcomes;
